Indications and Purpose of the Scan:  

  • CMR is an excellent way to non-invasively diagnose cardiac amyloidosis. CMR can help differentiate cardiac amyloidosis from other potential causes of a cardiomyopathy.

Why CMR (specific advantages):

  • CMR provides a non-invasive method to assess structure, function, and deposition of amyloid in the extracellular tissues via late gadolinium enhancement (LGE). CMR can also provide prognostic information for patients with a confirmed diagnosis.

  • Syed IS, Glockner JF, Feng D, Araoz PA, Martinez MW, Edwards WD, Gertz MA, Dispenzieri A, Oh JK, Bellavia D, Tajik AJ, Grogan M. Role of cardiac magnetic resonance imaging in the detection of cardiac amyloidosis. JACC Cardiovasc Imaging. 2010;3:155-164. This paper demonstrated that late gadolinium enhancement in cardiac amyloidosis is common and CMR can help identify cases where echocardiography is not definitive for CA.
  • Maceira AM, Prasad SK, Hawkins PN, Roughton M, Pennell DJ. Cardiovascular magnetic resonance and prognosis in cardiac amyloidosis. J Cardiovasc Magn Reson. 2008;10:5. This study followed a cohort of 29 patients with diagnosed cardiac amyloidosis. Though the presence of LGE was not in and of itself a significant predictor of mortality. However, death was predicted by gadolinium kinetics with 2 minute post-gadolinium T1 imaging.

Contraindications:

  • Any implanted device that is not MRI compatible.
  • Kidney dysfunction with a GFR <30 mg/dl/min)
  • Inability to lie flat
  • Inability to tolerate the scan
  • Altered mental status
  • Severe arrhythmias
