Level I
Level I training is intended to provide introductory knowledge in cardiovascular magnetic resonance (CMR). Level I competency represents knowledge of the indications for and capabilities of CMR, but not to the extent that is sufficient for independent practice and clinical interpretation of CMR. The topics to be covered in Level I training are listed in Table 1 at the bottom of this section.
Level I competency can be achieved by meeting the requirements of either Track A or B.
Track A is tailored for fellows or residents in training, and Track B is designed for physicians who have completed medical training.
The mentored cases and time spent in training in Track A in can also be applied towards meeting Level II certification requirements.
Track A (Formal fellowship or residency training)
It is recommended that all training programs (cardiology, internal medicine, radiology, pediatric cardiology, and nuclear medicine) incorporate basic training in CMR to familiarize the trainee with the indications, methods and applications of CMR.
Level I CMR competency can be achieved by meeting the following requirements, typically during the course of residency or fellowship training:
- A minimum of 1 month of cumulative (need not be consecutive) exposure to the methods and applications of CMR.
- Training must be under the aegis of a Level II or preferably Level III qualified mentor.
- Training must provide exposure to the topics listed in Table 1 below.
- Exposure to a minimum of 50 mentored cases.

Track B (Course*-based training)

This track is designed primarily for physicians who have completed medical training. Level I CMR competency can be achieved by meeting the following requirements, typically attained during a dedicated training course:
- The course must include a minimum of 2 full days of intensive, live instruction (either in person or remotely via a video-conferencing platform) in the indications, methods, and applications of CMR.
- The course director must be Level II or preferably Level III qualified.
- The course must cover all topics listed in Table 1 below. These topics may be presented as both didactic lecture and as case studies.

Level II
Level II training refers to the additional training in 1 or more areas that enables some cardiologists to perform or interpret specific procedures or render more specialized care for specific patients and conditions. This level of training is recognized for those areas in which an accepted instrument or benchmark, such as a qualifying examination, is available to measure specific knowledge, skills, or competence. Level II training in selected areas may be achieved by some trainees during the standard 3-year cardiovascular fellowship, depending on the trainees’ career goals and use of elective rotations. It is anticipated that during a standard 3-year cardiovascular fellowship training program, sufficient time will be available to receive Level II training in a specific subspecialty. In the case of CMR, Level II training is required for individuals who wish to perform and interpret CMR examinations as part of their practice of cardiovascular medicine.

Level III
Level III training requires additional training and experience beyond the standard 3-year cardiovascular fellowship to acquire specialized knowledge and competencies in performing, interpreting, and training others to perform specific procedures or render advanced specialized care at a high level of skill. In the case of CMR, Level III training would enable the trainee to direct a CMR laboratory or train others in CMR. As for other noninvasive imaging modalities, Level III training in CMR requires training in multimodality imaging.

Grandfathering
To provide a path to verification for established physicians to recognize their expertise in Cardiovascular Magnetic Resonance (CMR) imaging.
The expectation is that applicants have unquestionably supported the growth, awareness, and practice of CMR throughout their careers.
This recognition is particularly valuable for physicians looking to organize SCMR-endorsed educational activities around the world. A secondary, but important, objective is to support trainees by enabling verified and experienced CMR experts to serve as mentors and verification signatories.
Individuals who are eligible for Level II or Level III Verification, or who meet current SCMR Legacy criteria outlined in the 2018 SCMR training Guideline Document, should apply through those pathways.
Limited-Time Opportunity
The SCMR Grandfathering Pathway is a time-limited opportunity. All applications must be submitted by March 31, 2027.
Applications received after this deadline will not be considered.
Eligibility Criteria
Applicants must:
- Be established physicians who are not eligible for standard SCMR Level II or Level III Verification pathways and who do not meet current SCMR Legacy criteria.
- Have a minimum of 10 years of experience practicing CMR following completion of postgraduate medical training.
- Meet current Level III maintenance of competence expectations, including CMR-related CME and clinical case volume requirements.
- Demonstrate significant contributions to at least two of the following areas:
- CMR Education / Mentorship
- CMR Research
- Commitment to the Field of CMR
Individuals who qualify through existing SCMR Level II, Level III, or Legacy pathways should apply through those pathways instead.
Documentation Required
1. Cover Letter from Applicant Addressing:
- Summary of past training, clinical experience, and attestation that they do not qualify for Level II or III Verification through the standard pathways detailed in the SCMR Training Guideline Document (2018).
- Detail of the number of cases (not a case log) interpreted over the last two years. If fewer than 200 cases have been interpreted over the past two years, applicants should provide additional context, including clinical effort allocation (%) and the total number of cases interpreted over a longer period, to allow the committee to understand their overall CMR experience. A case log may be requested through random audit.
- Total hours of CMR-related CME completed over the last two years (it is acceptable to indicate “>40 hours”). If fewer than 40 hours of CMR-related CME have been completed over the last two years, applicants should provide additional information regarding their clinical effort and CME participation over a longer period. Supporting documentation may be requested through random audit.
- Description of how the applicant meets the eligibility criteria outlined above, including contributions to CMR education, research, and/or commitment to the field.
- Attestation that their medical license and staff privileges are in good standing.
2. Current Curriculum Vitae (CV)
The CV should clearly document the applicant’s professional experience and contributions to the field of CMR, including educational activities, mentorship, research achievements, leadership roles, publications, and other relevant accomplishments.
3. Board Certificate(s) or Equivalent
Applicants must provide copies of board certification(s) or equivalent credentials in their relevant specialty (e.g., Radiology, Adult Cardiology, Pediatric Cardiology, Nuclear Medicine).
4. Copy of Current Medical License
Applicants must provide a copy of their current and active medical license.
5. Attestation of Good Standing
Applicants must provide an attestation from their Division Chair, Department Chair, or Clinical Vice Chair confirming that their medical license and staff privileges are in good standing.
